SA Government launches Palliative Care Connect services

The South Australian Government’s Department for Health and Wellbeing’s End of Life Care team has officially launched Palliative Care Connect which provides information and linkages to palliative care and bereavement support in South Australia.

The Palliative Care Connect website, developed in partnership with Flinders University, will make it easier for South Australians, including healthcare professionals, to find relevant palliative care, bereavement and related community services. It provides key information about palliative care and bereavement, access to a service directory and a self-assessment tool designed to help individuals identify the support and resources they need.

Palliative Care Navigators, available through the phone line, provide information and service linkages, and empower individuals to make choices in alignment with their culture and preferences. To talk to a Palliative Care Navigator, call 1-800-725-548 (PALLI8), available Monday to Friday from 8.30am to 4.00pm. Voicemail is available after hours for a call back the following business day. People can also request a call back from a Navigator by filling out the call back form on the Palliative Care Connect website.
