Form	Requirements
Change of Program	For International Students who wish to change their program of study.
Course Add	This form can only be used after the last day to add a course online. It needs to be signed by the course coordinator.
Course Drop	This form can only be used by students who are not able to drop classes online.
Credit Transfer	This form is used to apply for credit for prior learning or work experience.
Cross Institutional	This form is for students who wish to take approved courses at the University of Adelaide for credit towards their study at their home institution.
Discontinuation of Studies	Discontinuation of studies form.
Honours Application Domestic International	Application for Honours for both BHSc(Hons) and Bachelor of Medical Science(Hons).
Leave of Absence	Application for Leave of Absence from program for 6 months or greater.
Offshore Enrolment	This form is for use by International offshore students who are unable to enrol online.
Refunds, Remissions and Re-credits	Students who withdraw from a course(s) after the Census Date due to special circumstances may apply for a Special Circumstances Refund.
Supp Exams	Application forms and policy for supplementary examinations for medical and compassionate reasons.
